Exciting, high profile, brand new contract in London Borough Hounslow.

We are a brand new team delivering exciting community based support service in London Borough Hounslow!

Commencing in January 2017, the Hounslow LIFE Integrated Support Service is an exciting new project that supports adults at risk of losing their independence through the delivery of personalised preventative support in the community.

We are looking for an inspirational Team Leader who will lead, motivate and empower a team of caseworkers to deliver community based, person-centred and holistic support services to people at risk of losing their independence

This is a great role for someone wanting to build their leadership skills in a brand new team and shape the service going forwards.

Person specification:

  • Experience of working with people who have a range of complex health, care and support needs in a person-centred, psychologically informed way
  • An understanding of how multiple needs overlap and create multiple disadvantage
  • A working knowledge of relevant legislation such as the Care Act 2014, Mental Capacity Act, Mental Health Act and welfare benefits / housing legislation
  • Ability to act as a staff champion, supporting colleagues, networking and building expertise and best practice within the team for your specialism
  • Thorough understanding of discrimination and disability issues, equal opportunities and anti- discriminatory practice
  • Ability to promote the service and give presentations to a variety of audiences

Location: Hounslow

Salary: £27,480 rising to £28,296 after 1 year

Grade: 29 – 30

Hours: 39 hours per week

Working Pattern: The service operates Mon – Fri from 8am – 9pm and Sat – Sun from 10am – 5pm so some flexi working to ensure contract is delivered within these times will be required.
